For your convenience, we accept Visa, MasterCard, Discover and CareCredit. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 847-605-8880. Many times, a simple telephone call will clear any misunderstandings. Please refer to our policy on insurance.

The fee will be discussed prior to the treatment. We request that the full fee be paid by the time our therapy is completed, either by partial payment at each appointment in the series or by a single complete payment prior to or at the last appointment. The cost of the final restoration by your general dentist is not included in our fee.

We are happy to help you receive the full amount to which you are entitled through dental insurance policies, by filling out forms, sending reports, etc., (when possible your claim will be submitted by computer). Although some offices charge an additional fee for filling out these forms, we have not found this necessary, but our agreement is with the patient and not with the insurance company. We ask that you pay for the therapy as outlined above and we will, in turn, see that you are reimbursed by the insurance company.

Policy of Insurance

At One Woodfield Place Endodontics we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this goal, we work hand in hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Patients often ask why we do not accept assignment of benefits from an insurance company. The actual reimbursement by an insurance carrier can vary a great deal not only between, but within insurance companies, often causing confusion and misunderstanding between patients and our office. Your reimbursement may not be equal to our fee. We have chosen to concentrate on your professional care rather than let such misunderstandings interfere with our office-patient relationship. Be assured we will do everything we can to make sure you are properly reimbursed by your insurance company. The following few paragraphs explain in detail how discrepancies and misunderstandings can occur.

Dental benefits vary considerably from one plan to the next, the range of benefits depending solely on what the purchaser wishes to offer employees or members. Some plans may cover as little as 30 percent or as much as 100 percent of dental services, with most falling in the 50 to 80 percent range. Some plans exclude certain types of services, i.e., orthodontics, or implants while other plans will cover a full range of dental services.

Some plans base the amount of benefit on a chart or schedule of fees arbitrarily developed by insurance companies. For this reason, you may receive a lower percentage of the reimbursement level indicated in your dental plan. For example, your plan may state that it will pay 80 percent of the fee arbitrarily determined by the insurance company and not 80% of the actual fee charged by us.

If you receive a communication from your insurance carrier suggesting that our fee is over and above the usual and customary rate for the services provided to you, please do not accept this as true without first discussing the matter with us. The insurance carrier’s fee data may be extremely out of date or not take into consideration local factors pertaining to the Chicago area in establishing its schedule. Our fees are well within the normal range for our area. If, after our discussions, you believe that the dental benefits provided by your plan are inadequate, you may want to discuss the matter with your employer, union, or association, so that alternatives can be investigated.

If your dental benefit plan requires a “pre-determination” or “prior authorization”, we will submit a treatment plan for review by your insurance carrier. However, please remember that the financial obligation for dental treatment is between you and this office. The insurance company is responsible to you and not us.

Again, we will help you in every way in filing your claims, handling insurance queries, processing follow-ups, lost claims, etc. No question is too small for you to ask, whether it is about our treatment, benefit plan or statement.
